Liability Waiver/Informed Consent Form

Group training is priced by package and agreed upon length of training. At the first session full payment or a completed payment contract with first installment is due. Scheduled training's are based on the group. Please call if you have to miss a session. Make-up sessions are not given.

I participate in the activity at my sole risk and responsibility.

I assume the risk of and responsibility for any injury or property damage resulting from my participation in the activity.

I state that I am physically able participate in any activity I choose and know of no medical or physical reason that participation could be potentially harmful in any way.

I release, indemnify, and hold harmless Advantedge Sports Training and its employees, trainers, and agents from and against any actions or claims which may be made by me or on my behalf in respect of, or arising out of any injury, loss, or damage caused to me or my property whether by my negligence,

breach of contract or in any way whatsoever.

This release of liability includes all injuries which may occur upon the result of use of any exercise equipment, products, training and classes: the sudden unforeseen malfunction of any equipment, instruction or supervision.

I hereby affirm that I am in good physical condition and do not suffer from any disability which would prevent or limit my participation in any programs offered by Advantedge Sports Training putting my health at risk.

I will report to the exercise specialist(s) any change in my physical condition (pain, soreness, injury, or new medications).

I acknowledge I have carefully read the release and I fully understand therein its

contents. I agree to give up any right to bring legal action against Advantedge Sports Training for negligence or any other personal injury, loss, or damage. I have read, understand, and fully agree with the release form for Advantedge Sports Training.

I authorize Advantedge Sports Training to record the image and voice of the subject named below and give Advantedge Training and all persons or entities acting pursuant to Advantedge Training's permission or authority, all rights to use of these recorded images and voice. I understand that said images and/or voice will be used for educational, advertising and promotional purposes in all conventional and electronic media, including but not limited to the internet, and any future media. I also authorize the use of any printed material in connection therewith. I understand and agree that these images and recordings may be duplicated, distributed, with or without charge, and/or altered in any form or manner without future or further compensation or liability, in perpetuity.
